• Internship Record and Evaluation Form—1 copy (to be approved by the company and the relevant engineer upon completion of the internship) (Click here for the form!)
• Internship Application Form—3 copies (Company Approval) (Click here for the form)
• Internship Placement Acceptance Letter – 1 copy (Company Approval) (Click here for the form)
• INFORMATION FORM ON UNEMPLOYMENT FUND CONTRIBUTION TO INTERNSHIP STIPENDS

• SGK SPAS Eligibility Certificate (Student) (To be obtained from the e-Government SPAS Eligibility Inquiry section)

• Students who will be completing an internship must also fill out the form linked below before receiving approval from the internship committee.
